Description

• As part of the application process for admission to JTC, women must complete a series of seven questionnaires from the Ontario Ministry of Health and Long Term Care
• The information from the questionnaires, together with the Individual Assessment Interview (see below), helps JTC staff assist you in making the most appropriate program decision

Location and timing:

• For women within commuting distance of the Centre: Sessions for completing the questionnaires are held at JTC's 215 Evans Avenue facility before the Phase I group on Tuesdays at 4:30 p.m.
• For women beyond commuting distance: Visit your local Addiction Assessment and Referral Centre, listed in the yellow pages, or contact the Drug Abuse Registry of Treatment at 1-800-565-8603; www.connexontario.ca
